The enrolment form takes around 20 - 40 minutes to answer.

 

Once you start the form, you won't be able to save it, so please make sure you have your details ready, including: 

 

1.  Your USI number, if applicable 

2.  Centrelink number, JSID, if applicable 

3.  Employment Service Provider or Employer Contact Details, if applicable

4.  Two forms of ID

5.  Medicare card

6.  Evidence of concessions/allowances, if required
